DocuSign Envelope ID:D8CCE739-BC40-475F-83D5-EB8CB65D9E58 <br /> [Departmental Use Only] <br /> TITLE DSME- FF <br /> FY 2016-2017 <br /> ORANGE COUNTY <br /> CONTRACT UNDER$1,000.00 <br /> NORTH CAROLINA <br /> THIS AGREEMENT, made and entered into this 1st day of July, 2016, ("Effective Date") by and <br /> between Orange County, North Carolina, a body politic and corporate organized under the laws of the State <br /> of North Carolina, (the "County"),party of the first part; and Functional Fitness, LLC (the "Provider"), party <br /> of the second part; <br /> WITNESSETH: <br /> For the purpose and subject to the terms and conditions hereinafter set forth, the County hereby <br /> contracts for the services of the Provider, and the Provider agrees to provide the following services to the <br /> County in accordance with the terms of this Agreement, time being of the essence: <br /> The services and/or materials and/or construction (hereinafter referred to collectively as "Services") <br /> to be furnished under this Agreement are as follows: <br /> 1. The Provider will teach the exercise component of group instruction for the Orange County <br /> Diabetes Self-Management Education(DSME)Program to include the following: <br /> Teach and instruct a one-hour exercise class following the objectives outlined in the NC Diabetes <br /> Self-Management Program Curriculum. <br /> Demonstrate safe and sound movement practices. <br /> Provide proof of liability insurance. <br /> Add additional educational components that are not in the curriculum but meet best practice <br /> guidelines and the Provider feels appropriate. <br /> Select additional handouts for the group presentation and provide updates to OCHD prior to the <br /> start of the class series. <br /> Request/choose educational teaching tools for OCHD to purchase for the class. <br /> 2. OCHD will: <br /> Provide the NC Diabetes Self-Management Program Curriculum for class outline <br /> development. <br /> Review the presentation and ensure that the components meet the NC Diabetes Self-Management <br /> Program Curriculum objectives. <br /> Compile presentation and handouts in participant notebook. <br /> Format the Provider's presentation in MS PowerPoint as updates are requested. <br /> Purchase educational teaching tools as the OCHD budget allows. <br /> Monitor instructor for safe and sound movement practices. <br /> Revised 6/16 1 <br />
