• All UNC Family Medicine and Wellness @Work personnel have had training and <br /> education concerning patient privacy and requirements under the HIPAA Privacy <br /> Regulations. <br /> • All health information obtained during health screening and coaching sessions <br /> will be stored within our secure UNC systems according to federal privacy <br /> regulations. <br /> • Participation in the health screening and health coaching program is strictly <br /> confidential. <br /> • Health screening areas will be arranged in such a way that ensures patient privacy. <br /> • Health coaching will be provided in a private, enclosed space (conference room, <br /> office, etc)to be provided by the employer. <br /> • No personal or identifiable health information will be shared directly with the <br /> employer, or anyone outside the UNC Family Medicine Wellness @Work clinical <br /> staff. <br /> • All information shared with the employer will be de-identified and reported in <br /> aggregate. <br /> • A process for outreach to at-risk employees that ensures confidentiality will be <br /> developed in collaboration with employer. <br /> Responsibilities of County <br /> • Secure appropriate space for health screening/coaching <br /> • Provide a designated point of contact to assist with planning and logistics <br /> • Provide incentives to encourage employee participation <br /> • Assist with the distribution of marketing and promotional materials to employees <br /> • Work with our staff to determine preferred risk levels and number of coaching <br /> sessions <br />
