Union St. <br />City: Hillsborough, N.C. 27278 <br />Phone Number: 732-3173 <br />Date: - October 9, 2006 <br />Note: Prior to conducting Lead-Based Abatement activities, all personnel associated <br />with the project should review and fully understand the requirements of ALL <br />applicable local, state and federal regulations that may apply to the worksite. <br />Worksite Preparation: <br />Ground Containment: <br />Before construction, construct a worksite perimeter curb of <br />2x4 timbers, ten feet from the house exterior. Create an outer <br />barrier of flags and/or plastic tape 3 feet on center and 20 feet <br />from the worksite. All warning signs must remain posted until <br />clearance is passed. ( Both exterior door frames) <br />2. During the period of work on the exterior door frames, attach a <br />layer of 6-mil poly to the door opening to prevent lead dust and <br />paint chips from migrating into the unit. <br />Pre-Clean: <br />Pre-clean the location ( window sill) in room # 5 ( rear right bedroom) <br />Refer to the Risk Assessment / Lead Paint Inspection- HEPA vacuum <br />this location to remove all lead dust. <br />Clearance Examination: <br />1. After completion of all work that disturbed LBP and any other lead <br />hazard reduction activities, a qualified person shall perform a <br />clearance examination. Work disturbing lead-based paint is not <br />considered complete until clearance is achieved.
