DocuSign Envelope ID:276E5C1E-2398-4C5A-BAC3-4D588BFC7983 <br /> Exhibit A <br /> Scope of Work: FSA Career Explorers 2016 <br /> Empowerment, Inc. will: <br /> Provide the Career Explorers program, an 8 week work experience, to 10 youth aged 16-21 in Zones 4 <br /> and 6 of the Family Success Alliance. The program includes a supervised position at a local employer <br /> for 35 hours per week, and a 5 hour group session on Friday afternoons from 12-5 PM that focuses on <br /> financial skills,youth development, and preparation for college and career. The program will run from <br /> June 20th to August 12th 2016. <br /> To accomplish this, Empowerment, Inc. will: <br /> 1) Recruit 10 low-income youth aged 16-21 who live within the boundaries of Zone 4 or Zone 6, <br /> and/or attend a target zone high school (Orange High, Cedar Ridge, and Carrboro, Chapel Hill <br /> and Phoenix Academy) to participate in the Career Explorers by June 15th, 2016. <br /> 2) Recruit 10 employers that agree to provide a supervised work experience by June 15th, 2016. <br /> 3) Identify a Career Explorers Coordinator responsible for communicating with youth participants, <br /> employers, and staff of Boomerang and Family Success Alliance. <br /> 4) Work with Boomerang staff to schedule all necessary materials for Friday sessions, to be held at <br /> Boomerang's offices. EmPOWERment staff will also arrange for transportation for these <br /> sessions. <br /> 5) Be responsible for all payment of participants and program supplies. <br /> 6) Report mid-program program attendance and qualitative report on program successes and <br /> challenges to the Family Success Alliance Program Manager by July 15, 2016 (this date <br /> represents the midway point of the program). Report final program attendance and evaluations <br /> to Family Success Alliance Program Manager by September 15, 2016. This includes an <br /> employer evaluation of participants and the program, and an evaluation for participants to <br /> complete at the start and end of the program. The participant evaluation will be created together <br /> with Family Success Alliance evaluation staff to ensure alignment with overall project goals and <br /> evaluation. <br /> 7) Follow-up with program participants 6 and 12 months after the end of the program and report <br /> outcomes to the Family Success Alliance Program Manager by February 15, 2017 and <br /> September 2017. <br /> Family Success Alliance staff will: <br /> 1) Provide assistance determining the eligibility of applicants' addresses, as requested by <br /> Empowerment staff. <br /> 2) Provide technical assistance in the creation of the participant pre/post survey tool. <br /> 3) Connect the Career Explorers Coordinator with existing transportation systems to explore for <br /> Friday transport. <br /> 4) Assign at least one FSA Zone Navigator to attend Friday sessions. <br />
