Responses that do not comply with all requirements may be <br /> considered non-compliant. Offeror's signatory must review each item below, and certify by initialing in the space <br /> to the right. <br /> This Response Includes: Init. <br /> 1 An"Original" of all required FORMS and documents,plus one copy, each in a separate hard-sided 3- <br /> ring binder. <br /> 2 A completed electronic copy of ALL Forms, and any other documents required by this Invitation to <br /> be submitted electronically, on electronic media, e.g. CD-ROM. Forms D & E are to be provided in <br /> an excel format. <br /> 3 All required Form A's have been submitted with the bid package (Dealer, Mfg., Converter <br /> Texas Commercial Chassis/vehicle dealers) where applicable. <br /> 4 The required list of References. <br /> 5 A complete description of Offeror's "Service Organization", detailing support locations, hours, <br /> personnel and parts/service availability. <br /> 6 Complete Warranty Documentation for all Products offered. <br /> 7 If the Non-Resident Reciprocal Bid Act applies, a copy of your state statute and a determination of <br /> the status of Texas bidders/proposers in your home state. If not applicable, indicate "N/A" <br /> 8 If offer includes motor vehicles to be sold in Texas, copies of all current licenses as required by the <br /> Texas Motor Vehicle Commission Code (ambulance, converter and trailer licenses) where <br /> applicable. <br /> 9 The manufacturer's latest Specification Documents detailing standard features, operating <br /> characteristics, etc., for each Base Product Item offered. Note: Bidder shall supply a spec/quote <br /> sheet for each product code bid on Form D. These sheets should detail what features are <br /> included in bid unit the quoted on Form D. <br /> 10 Form CIQ, if required by law, completed and signed. <br /> Go to http://www.ethics.state.tx.us/whatsnew/conflict—forms.htm to get instructions and download the <br /> Form. <br /> 11 A Bid/Proposal Bond, or Cashier's Check in the amount of $3000.
