11 <br /> Section 4. Evaluation. <br /> Section 4.1 Funded grants shall be evaluated by the Monitoring Committee. <br /> Section 4.2 No JCPC member who is paid by or directly supervises someone paid by <br /> local grant funds, may serve on the Monitoring Committee. <br /> ARTICLE VI: CONFLICT OF INTEREST <br /> Section 1. The JCPC Chairperson shall have all JCPC members complete and sign <br /> the disclosure form(form DJJ 13 OOlc) and inform all members of the <br /> conflict policy and review annually. <br /> Section 1.1 All members must disclose in writing the existence, nature and extent of <br /> any potential or actual conflict of interest using (form DJJ 13 001c). <br /> Section 1.2 If a member directly or indirectly benefits from the disbursement of these <br /> state funds. They will not serve on the monitoring or funding committees, <br /> nor be able to vote re: funding decisions. <br /> Section 2 If a conflict is discovered after the fact, the JCPC Chairperson will inform <br /> the JCPC of the conflict of interest. <br /> Section 3 All conflict of interest disclosure forms must be filled out by all members. <br /> ARTICLE VII: EMAIL VOTING <br /> Section 1. The Initiator <br /> Section 1.1 Any active member or JCPC Coordinator can initiate mail voting by sending <br /> voting proposal via email. <br /> Section 1.2 The member who has initiated a vote can retract the voting proposal by sending <br /> an email to the membership list stating the intention to retract the proposal. <br /> Section 2. Email Voting Process <br /> Section 2.1 Email voting proposal is initiated by sending an email to the entire membership <br /> mailing list containing the words "Voting Proposal" in the subject line and <br /> explicitly stating the subject of the voting. <br /> Adopted June 2015 <br />
