Exhibit A <br /> Scope of Services for <br /> Orange County Schools "Out of School Time" Summer 2016 programs <br /> forthe <br /> Family Success Alliance <br /> Purpose: Program support resources for Orange County schools to provide out of school time <br /> services to Family Success Alliance (FSA) students attending New Hope Elementary, a zone 4 <br /> school. <br /> Budget: $41,745 <br /> • $22,770: Summer K-Readiness program for 30 FSA students at a cost of$759 per <br /> student. <br /> • $18, 975: Summer Rising 15Y graders Literacy Enrichment program for 25 FSA students at <br /> a cost of$759 per student. <br /> Funds will be used to pay teacher salaries and payroll, transportation, curriculum materials and <br /> supplies. <br /> Program Services: <br /> • Recruit and enroll 30 incoming kindergarteners at New Hope Elementary into a summer <br /> kindergartern readiness program. Recruit and enroll 25 rising 1 grade students into a <br /> summer literacy enrichment camp. <br /> • Provide a four week program for students (16 program days) to prepare students to <br /> enter school at grade level using an evidenced-based curriculum and an 8:1 teacher <br /> student ratio. <br /> • Provide a program orientation for parents, two parent workshops and an end of <br /> program celebration. <br /> • Provide academic assessment at the beginning and end of the program to evaluate the <br /> impact of the program and collect intergrated health assessment data. <br /> Outcomes <br /> K-Readiness <br /> Recruitment: Staff, with assistance from FSA staff, will conduct outreach to eligible families and <br /> recruit up to 33%of incoming kindergarten students at New Hope Elementary school for the k- <br /> readiness program for a total of 30 students. <br /> Attendance: Attendance will be tracked with a goal of 90% attendance rate. <br /> Integrated health assessments: School staff will collect standardized assessments as agreed to <br /> with OCHD and conduct referrals as needed. <br /> 1 <br />
