DocuSign Envelope ID: D666A894-BFFB-4111-8D4B-404A52D1 3C1 1 <br /> Attachment A <br /> Summer 2016 Practicum Scope of Work for Marisa Martini <br /> marisamPlive.unc.edu <br /> 1) Caregiver Support Senior Resource Team (SRT) Plan: Marisa will work with <br /> the Caregiver Support SRT and its "Senior Leader" volunteers to develop and <br /> implement a comprehensive plan for the team to support Orange County <br /> caregivers, including those caring for family members with dementia, chronic <br /> disease, or simply growing old. Tasks will include attending and participating in <br /> monthly meetings, providing technical support to team (e.g.,taking notes and <br /> meeting minutes, and organizing volunteers around caregiver support projects), <br /> and programmatic support (working with staff and volunteers to develop <br /> project plans). The final deliverable will be a program plan focusing on one or <br /> two specific projects,with action steps and a timeline for implementation. <br /> 2) UNC Partnerships in Aging Program (the Partnership) Caregiver Needs <br /> Assessment: In collaboration with the Partnership, Marisa will conduct a needs <br /> assessment of UNC, Chapel Hill, faculty, students, and staff to identify the <br /> number and types of caregivers to older adults, as well as their needs around <br /> caregiving support and resources. The final deliverable will be a needs <br /> assessment report, outlining the methods and results of assessment. <br /> 3) Master Aging Plan (MAP) Senior Housing Needs Focus Group Report: In <br /> collaboration with other students and MAP committee members, Marisa will <br /> participate in the MAP planning process by developing a questionnaire for, and <br /> conducting, a focus group around senior housing needs and desires for a <br /> potential Habitat for Humanity Orange County senior housing development. The <br /> final deliverable will be a report, outlining the methods and results of the focus <br /> group. <br />
