DocuSign Envelope ID:7190449A-3B32-4622-B7F4-C359EAAB94BA <br /> Exhibit A-Amendment to Scope of Services <br /> Additional Project Scope: <br /> MHAT will provide supervision to the 4 members of the Family Success Alliance zone navigator <br /> team through bi-monthly group supervision sessions, in-service training, individual <br /> consultations, shadowing other workers, and coaching on fulfilling the Zone Navigator role. <br /> Goal: Zone Navigators will demonstrate the skills needed to conduct effective home visits with <br /> clients. <br /> • Provide protocols to prepare for and conduct home visits <br /> • Review safety strategies for home/community work with families <br /> • Zone Navigators will accompany/shadow FAN staff on home visits/client intakes <br /> • Accompany Zone Navigators on home visits/intakes, observe and provide coaching and <br /> feedback <br /> Goal: Zone Navigators will demonstrate an understanding of how to maintain personal and <br /> professional boundaries with families and of the impact of self-awareness and self-disclosure <br /> when working with families. <br /> • Use case reviews as a framework to explore the principles of boundaries, self- <br /> awareness, self-disclosure and self-care during group supervision <br /> • Provide opportunities for professional development through in-service training, relevant <br /> articles for discussion, and community agency site visits <br /> Goal: Zone Navigators will help families build protective factors and increase resiliency in their <br /> lives and communities. <br /> • Provide training on the framework of Resiliency (Risk vs Protective Factors) <br /> • Provide tools for discovering existing strengths and protective factors with families <br /> • Identify protective factors needed to support individual and family resilience, and <br /> strategies for increasing protective factors <br />
