Oni Health Reporting <br /> FY 15-16 <br /> ORANGE COUNTY <br /> CONTRACT UNDER$15,000.00 <br /> NORTH CAROLINA <br /> THIS AGREEMENT, made and entered into this 15ffi day of January, 2016, ("Effective Date")by <br /> and between Orange County, North Carolina, a body politic and corporate organized under the laws of the <br /> State of North Carolina, (the "County"), party of the first part; and Adebukola Om (the "Provider"), party of <br /> the second part; <br /> WITNESSETH: <br /> For the purpose and subject to the terms and conditions hereinafter set forth, the County hereby <br /> contracts for the services of the Provider, and the Provider agrees to provide the following services to the <br /> County in accordance with the terms of this Agreement, time being of the essence: <br /> The services and/or materials (hereinafter referred to collectively as "Services") to be furnished <br /> under this Agreement are as follows: Complete formative research on the state of the relationship between <br /> Orange County Health and carceral systems to 1) Identify unmet physical and mental health needs of <br /> incarcerated and formerly incarcerated people; 2) Identify challenges to addressing those health needs; 3) <br /> Identify key players engaged in addressing those needs; 4) Identify model policies/practices that are helping <br /> to meet those needs and 5) Recommend appropriate programming to help mitigate those needs (see Exhibit <br /> A: "OCHD Research and Report Writing Work Plan.") <br /> Deliverables are as follows: <br /> Deliverable 1: Written report on local incarceration data and survey of state and national data on <br /> model policies and practices by January 30, 2016. <br /> Deliverable 2a: Notes from 5 key informant interviews of Orange County stakeholders connected <br /> with the carceral and health systems by February 28, 2016. <br /> • The interview guide will be developed by Meredith Stewart,Program Manager, and provided to <br /> Ms. Om for feedback and use. <br /> • A list of potential key informants will be provided by Meredith Stewart to Ms. Om for <br /> discussion and final choice. <br /> • Ms. Stewart will provide e-mail introductions between Ms. Om and the key informants. Ms. <br /> Om will be responsible for scheduling and conducting the interviews after this introduction. <br /> Deliverable 2b: Written report on emergent themes and core meaning from key informant interviews <br /> by March 30, 2016. <br /> Deliverable 3a: By April 22, 2016, the first draft of a written report synthesizing the local data, <br /> model practices/policies, and key informant interviews to show 1)the major health <br /> concerns/challenges for people involved with the carceral systems in Orange county, 2) gaps in the <br /> Orange County system as compared to model policies/practices, and 3)recommendations for priority <br /> policies/practices based on this information. <br /> Deliverable 3b: By May 16, 2016, the final draft of the report described in Deliverable 3a. <br /> Deliverable 4: By May 2015, a presentation to the Board of Health Mental Health& Substance <br /> Abuse Subcommittee of the findings of the report. <br /> Revised 10/14 1 <br />
