Help determine the need for specific levels of housing assistance Rental or Homeownership <br /> 5. Assist households identify and obtain housing that is affordable in Orange County <br /> specifically. <br /> 6. Provide a yearly report to the Orange County Housing, Human Rights and Community <br /> Development office, with an assessment of the current needs in the community addressing <br /> rental housing, support housing and permanent housing. <br /> Measurements and Outcomes Expected <br /> 1. Outreach and Engagement: This will include an active phase of outreach and engagement <br /> to community action groups, landlords, realtors, local government and outside referral <br /> agencies who provide rental and residential housing in Chapel Hill, Carrboro, Hillsborough <br /> and surrounding areas. Plan and conduct three outreach activities in coordination with <br /> HHRCD by June 30, 2016. <br /> 2. Housing Stabilization: For prevention and other housing activities, case managers will <br /> provide considerable skill and community knowledge. They will explore the skills, <br /> resources, and abilities needed to support effective identification, assessment,prevention, <br /> fe-housing, stabilization, and support. Complete assessments of and assist 25 clients with <br /> goal setting and their plan development with five clients/tenants on track to achieve <br /> homeownership by December 31, 2016. <br /> 3. Helping Clients Identify Short-Term and Long-Term Goals: CHOEDC offers short-term <br /> and medium-term assistance; however, many clients have long-standing significant issues <br /> that should be addressed in long-term case management. The case manager will focus on <br /> resolving immediate needs, while creating linkages to other systems that support long-term <br /> housing stability. Provide linkages to other long term support for five or more <br /> clients/tenants as needed and determine progress and whether stabilized six months later <br /> (June 30, 2016). <br /> Monitoring Progress <br /> The overarching goal is to establish long-term housing stability for clients/HCV tenants in <br /> particular and for other residents in need of affordable housing in general in Orange County. The <br /> CHOEDC housing staff will follow up with the Orange County Housing, Human Rights and <br /> Community Development Department and tenant to ensure that the case management process has <br /> helped to resolve or eliminate the housing issues that are prevalent in their individual lives and <br /> how they have progressed toward their goal. <br /> (Centre for HomeOwnership and Economic Development) <br /> Orange County Outside Agency Performance Agreement Page 9 of 7 <br /> Rev. 6/15 <br />
