Individual program level evaluation plans for the 1) FSA Kinder Readiness <br /> Programs and 2) Zone Navigators. <br /> a. Meet with FSA staff to review individual program goals, objectives, <br /> activities, and outline priority evaluation questions by October 31, <br /> 2015. <br /> b. Research and identify 3 to 5 best practice process and outcome <br /> measures per program to provide to FSA staff by December 1, 2015. <br /> c. Identify additional indicators and the frequency with which to <br /> measure program implementation, key child and family demographic <br /> information for longitudinal tracking purposes by December 1, 2015. <br /> d. Research and provide available, validated tools for selected process <br /> and outcome measures by December 1, 2015. <br /> 2. Longitudinal evaluation plan - Provide a written document to serve as a <br /> roadmap for use by FSA staff to develop MOUs with key partners (including <br /> the schools, community non-profits, etc.) as well as a common consent <br /> form. The document will include important points to discuss with the <br /> school districts around FERPA; and information regarding the potential use <br /> of the NC Education Data Research Center. <br /> a. Meet with FSA staff to review program goals, objectives, activities, <br /> and outline priority annual and longitudinal evaluation questions by <br /> October 31, 2015. <br />
