<br /> NORTH CAROLINA CONSTRUCTION SERVICES <br /> AGREEMENT UNDER$50,000 <br /> ORANGE COUNTY <br /> THIS CONSTRUCTION AGREEMENT (hereinafter"Agreement"), is made and entered <br /> into this 16 day of September, 2015 by and between Orange County,North Carolina(hereinafter <br /> the "Owner") party of the first part; and Four Star Petroleum Services, Inc. (hereinafter the <br /> "Contractor"),party of the second part; <br /> WITNESSETH: <br /> For the purpose and subject to the terms and conditions hereinafter set forth, the Owner <br /> hereby contracts for the construction services of the Contractor, and the Contractor agrees to <br /> provide the construction services to the Owner in accordance with the terms of this Agreement. <br /> 1. TERM <br /> Beginning and ending dates of contract: September 16, 2015 through December 15, 2015. <br /> The Project Commencement Date shall be September 21, 2015, or as soon as scheduling can take <br /> place. <br /> 2. MAXIMUM AMOUNT PAYABLE <br /> Dollar Amount Not to Exceed: Thirty Five Thousand Eight Hundred Fifty Five Dollars and <br /> Thirteen Cents ($35,855.13) <br /> 3. SERVICES <br /> Contractor agrees to provide the following construction services(the"Work"): <br /> • Pull all permits and bill the cost back to the owner. <br /> • Remove existing AST equipment off of the tank and relocate to a new site. <br /> • Excavate a pad area 3 Px 15'x 8"and form with#8 rebar on 18"centers both ways. <br /> • Set 16 (sixteen) 6"x 7' steel bollards on 3' centers. <br /> • In the excavated area,use 4,000 PSI concrete and saw cut joints within 24 hours. <br /> • Bollards to be filled solid with concrete and painted. <br /> • Arrange for 150 Ton Crane to move existing 8,000 gallon tank off of the existing pad and <br /> place on a trailer to be moved approximately a 16th of a mile on the designated property. <br /> • Set the tank on the new excavation area using the same crane. <br /> • Air test tank at 3 psi for one hour. <br /> • Reinstall existing equipment back on tank as found. <br /> • Owner to fill tank to 90%capacity. <br /> • Excavate trench for power line approximately 140' over to the existing building from the <br /> tank pad. <br /> • Backfill trench with materials removed. <br /> • Run new conduit and wire to designated existing breaker panel (approximately 160'). <br /> • Wire existing suction pump and new solenoid valve and the new emergency stop. <br /> • Startup system and check for proper operation. <br /> • Clean any debris up created by crane company or Four Star Petroleum. <br /> Contractor shall not sub-contract all or any part of the construction services provided for in <br /> this Agreement without prior written approval of the Owner.
