DocuSign Envelope ID:052023BB-2927-4787-B6F4-A76E64212B03 <br /> ORANGE COUNTY <br /> AMENDMENT TO SERVICES AGREEMENT <br /> NORTH CAROLINA <br /> THIS AMENDMENT, made and entered into this the 25th day of Septemmber, 2015, by and between the County <br /> of Orange, a body politic and corporate of the State of North Carolina, ("County"), and Courtney Kennedy <br /> ("Provider"); <br /> WITNESSETH: <br /> WHEREAS, the County and Provider entered into a Services Agreement dated, July 1, 2015, to provide services to <br /> be rendered by Provider to County's Health Department("Original Agreement"); and <br /> WHEREAS, the County and Provider desire to amend the Original Agreement, while keeping in effect all terms <br /> and conditions of the Original Agreement not inconsistent with the terms and conditions set forth below. <br /> NOW THEREFORE, for and in consideration for the mutual covenants and agreements made herein, the parties <br /> agree to amend the Original Agreement as follows: <br /> Section 4 Duration of Services <br /> a. Term. The term of this Agreement shall be from July 1, 2015 to December 31, 2015. <br /> Section 5. Compensation <br /> a. Compensation for Basic Services. Modify the sentence as follows: The maximum amount payable for <br /> Basic Services shall not exceed Twenty-Nine Thousand Seven Hundred Dollars ($29,700). <br /> d. Travel. Modify the sentence as follows: Travel reimbursement shall not exceed Seven Hundred Fifty <br /> Dollars ($750). <br /> e. Supplies. Add the following sentence: Provider may be reimbursed up to an additional One Hundred <br /> Twenty-Five dollars ($125) for necessary project-related expenses during the period October 1 through December <br /> 31s` provided that Provider demonstrates that she has expanded the monies previously distributed to her for <br /> supplies. <br /> Except for the changes made to Sections 4.a., 5.a, 5.d., and 5.e. herein, the Original Agreement shall remain in full <br /> force and effect to the extent it is not inconsistent with this Amendment.
