DocuSign Envelope ID:8B670D51-A7FD-4694-8E94-963553021305 <br /> NORTH CAROLINA ENDORSEMENT <br /> MANAGEMENT & PROFESSIONAL LIABILITY POLICY <br /> This Endorsement is subject to the Declarations, Terms, Conditions And Other Provisions, except as <br /> modified in this Endorsement. <br /> Claims Information <br /> The Claims Information Condition is added to the Terms, Conditions And Other Provisions as follows: <br /> Within 45 days after the mailing or delivery of a written request from the "insured organization," the <br /> "insurance organization" shall mail or deliver the following loss information covering a 3-year period: <br /> a. Aggregate information on total closed "claims," including date and description of "loss," and any <br /> paid"loss"; <br /> b. Aggregate information on total open "claims," including date and description of"loss," and amount <br /> of any payments; and <br /> c. Information on notice of any"claim,"including date and description of"loss." <br /> Deductibles And Limits Of Liability <br /> The Deductibles And Limits Of Liability Condition in the Terms, Conditions And Other Provisions is <br /> replaced with the following: <br /> 1. Deductibles <br /> The "insurance organization's" liability with respect to "loss" arising from each "claim" shall apply <br /> only to that part of"loss" that is excess of the applicable deductible shown in Item 3. (C) or Item 8. <br /> (C) on the Declarations. <br /> If "loss" arising from a single "claim" is subject to more than one deductible, the applicable <br /> deductible shall be applied separately to each part of such "loss," but the largest applicable <br /> deductible shall be the maximum deductible applicable to all "loss" arising from such single "claim." <br /> If a single deductible applies to multiple "insureds," the deductible shall be pro-rated among such <br /> "insureds." <br /> 2. Limits Of Liability <br /> For all coverages included in this Policy as shown in Item 3. (A) on the Declarations that are made <br /> part of the Policy Annual Aggregate Limit Of Liability shown in Item 3. (B) on the Declarations, the <br /> Policy Annual Aggregate Limit Of Liability shown in Item 4. on the Declarations is the maximum <br /> amount the "insurance organization" shall be liable to pay, for all "loss" resulting from all "claims" <br /> first made during the "policy period." <br /> MPL 0020 07 13 NC CUMIS Insurance Society, Inc.
