It includes an overview of the project,a detailed description of <br /> the responsibilities that we share, important contractual clauses, and a statement of agreement that <br /> requires your signature. Once signed by both parties,this letter of agreement will serve as a <br /> contract between Americans for the Arts and the Orange County Arts Commission for the period <br /> beginning on the date of signature and ending June 30,2017. <br /> TO CONFIRM YOUR STATUS AS AN OFFICIAL STUDY PARTNER: <br /> 1. Initial each of the SIX required tasks to be completed by the Orange County Arts <br /> Commission(listed on Pages 4-10 of this document); <br /> 2. Choose from the optional study"add-ons" offered to all study partners(listed on Page 9 <br /> of this document); <br /> 3. Sum the total contract fee for the Orange County Arts Commission, including your base <br /> study participation fee plus the separate fees for any of the"add-ons"that have been <br /> chosen(at the bottom of Page 9); <br /> 4. Choose the payment schedule for your study participation fee(on Page 10); <br /> 5. Approve the Contractual Clauses by initialing the bottom of Pages 14 and 15; <br /> 6. Sign and date the Statement of Agreement on Page 16; and <br /> 7. Return the entire completed contract to Ben Davidson at Americans for the Arts(either <br /> mail the signed hardcopy or email a scanned PDF of the signed contract). <br /> 8. Upon receipt, an Americans for the Arts representative will sign the contract, and we will <br /> return a copy to you electronically. <br /> We recommend that you keep a copy.We also recommend that you don't delay—we only <br /> guarantee participation for the first 200 local study partners that remit a signed contract. <br /> 1 <br />
