Expense Description Amount Scheduling staff salary: coordinating schedules, interviewing and training volunteers $1,200 Overhead costs for volunteer recruitment and vetting: marketing,background checks, etc. $600 Insurance costs for volunteer companions: liability, etc. $200 Program Services For assistance with this or the following section,please reference the Exhibit A instructions and example,located within the contract and reporting memorandum.Outline the major activities the agency will employ to attain the Anticipated Outcomes below,by June 30,2016. • Provide 100 total hours of companion services for Orange County seniors who would otherwise be unable to afford this vital service. • Charity Care Program: Provide eligible individual seniors with up to three hours per week of companion care; which includes transportation to medical appointments, food shopping, and light housekeeping • Combat social isolation in low-income senior residents of Orange County through companion services which include social outings,reminscing exercises and crafts. Anticipated Outcomes The Anticipated Results column must include quantifiable results in the form of number of persons/units served within Orange County,only(all Towns and municipalities).If you use percentages,you must also provide the total number of participants within that measure's description or for an earlier performance measure.
