An outreach plan with action steps to disseminate information to medical providers <br /> serving older adults in Orange County.The purpose of the information dissemination <br /> plan is to inform primary care providers and other medical specialists about community- <br /> based services provided by the Department on Aging that can support the care of older <br /> adult patients in the home setting. <br /> 2. Literature review citing innovations in engaging physicians in 1. Inter-professional <br /> teamwork within new care delivery models (ACO's, Patient-Centered Medical Homes) <br /> and 2. Referral to community-based health services <br /> 3. A program plan for prevention and primary care service delivery within the Senior <br /> Centers <br /> Tasks will include: <br /> • Identifying local providers and sorting by medical network (UNC, Duke, etc.) <br /> • Determining the best ways to educate various providers about services provided by the <br /> Orange County Department on Aging and how to access them <br /> • Pilot testing selected outreach methods <br /> • Developing recommendations for future action <br /> • Organizing and participating in formative meetings between Department on Aging staff <br /> and UNC Physicians <br />
