EXHIBIT"A" <br /> Scope of Services—FY 2014- 15 <br /> Outside Agency Performance Agreement <br /> Agency Name: Chapel Hill Carrboro City School <br /> Program Name: Kindergarten Jump Start <br /> Funding Award: $50,200 <br /> Outline how the agency will spend Orange County's funding award. <br /> Expense Description Amount <br /> Staff--Includes 4 teachers for an 8:1 student ratio for 16 full teaching days and 3 teacher $40,000 <br /> workdays for prep time and one site coordinator paid for 20 fulltime workdays to include <br /> prep and program hours <br /> Transportation--School bus driver and mileage for Carrboro&FPG Elementary students 5,000 <br /> Curriculum and Supplies: Will include an evidenced-based curriculum and supplies $2,000 <br /> including copy paper,photocopying,markets,craft supplies <br /> Meals: (will be provided as part of the free and reduced lunch program) $0 <br /> Parent Engagement materials: Food,childcare, interpretation services,workshop $2,000 <br /> materials <br /> Student Recruitment and Supplies: Books,backpacks,other back to school supplies 1,200 <br /> Program Services <br /> For assistance with this or the following section, please reference the Exhibit A instructions and example, located within the <br /> contract and reporting memorandum. Outline the major activities the agency will employ to attain the Anticipated Outcomes <br /> below,by June 30,2015. <br /> • Recruit and enroll 30 incoming kindergarteners at Frank Porter Graham Elementary and recruit <br /> and enroll 30 incoming kindergarteners at Carrboro Elementary into a summer kindergartern <br /> readiness program. Incoming kindergarteners from Northside Elementary school can attend the <br /> Carrboro program as slots exist. <br /> • Provide a three or four week program for students to prepare them to enter kindergarten, 15-16 <br /> program days total. <br /> • Provide academic assessment at the beginning and end of the program to evaluate the impact of <br /> the program and collect intergrated health assessment data. <br /> Anticipated Outcomes <br /> The Anticipated Results column must include quantifiable results in the form of number of persons/units served within Orange <br /> County, only (all Towns and municipalities). If you use percentages, you must also provide the total number of participants <br /> within that measure's description or for an earlier performance measure. <br /> Performance Measures Anticipated <br /> Results <br /> Recruitment: Staff will conduct outreach to eligible families and recruit up to 33%of 30 <br /> incoming kindergartners at FPG and Carrboro Elementary school. <br /> Attendance: Attendance will be tracked with a goal of 90%attendance rate 27 <br /> Integrated health assessments: Staff will collect kindergarten health assessments and 27 <br /> conduct behavioral health screenings or referrals as needed. <br /> Certified by: !=n A 64r 5 a-v\ °`i)ate: 0-014' S <br /> r vider's Signature <br />
