DocuSign Envelope ID:48959198-4FAC-490E-95C3-5A542B2E055D <br /> The Orange County DSS and the partners of the collaborative agree to maintain client files and make <br /> them available to UWGT for monitoring, research or evaluation purposes.At a minimum,files should <br /> contain demographic information about the client, identified client goals/outcomes and progress on <br /> those outcomes, and documentation of expenses associated with each of these clients for education, <br /> employment and housing related activities. <br /> UWGT will support this collaborative by developing an integrated or shared data collection and <br /> reporting system that enables partners to target interventions and strategies in a way that ensures <br /> maximum impact and success. The partners will implement procedures to request permission to share <br /> data amongst themselves and with the UWGT. <br /> 6. Reimbursement <br /> UWGT will provide reimbursement based funding in the amount of$ 60,000 for the purposes of housing <br /> stabilization,educational and employment activities, and financial literacy and planning. The Orange <br /> County DSS shall submit by the 3rd day of each month a billing statement that consists of a tracking <br /> spreadsheet.The Orange County DSS will be reimbursed for salary cost of Life Skills Foundation staff <br /> time provided for each youth served during the previous month and any reimbursements requested for <br /> housing,employment, education, and transportation identified for the previous month.-UWGT will <br /> provide reimbursement by the last week of the month the submission was made. UWGT is not <br /> responsible for ensuring that the collaborative partners are receiving their appropriate or timely <br /> payment. <br /> 7. Amendments <br /> This Agreement may be amended at any time upon mutual written and signed agreements of Orange <br /> County DSS and UWGT <br /> 8.
