Performance Monitoring/Quality Improvement <br /> This project is a pilot of a new service delivery model for a specific target population. UWGT will be part <br /> of ongoing support to meet the challenges of the implementation of this new collaborative model. <br /> Efforts will be made to meet with the partners of the collaborative on a regular basis in order to <br /> promote communication and trust amongst the partners. <br />
