Roles and Responsibilities Within the Partnership Partner Roles Orange County + Act as the lead agency within the collaboration Department of • Attend and participate in meetings with the Collaborative at a frequency Social Services determined appropriate by the partner agencies ($32,000) • Provide primary case management for all participants through the LINKS program. • Ensure each participant has access to eligible LINKS funding to promote the achievement of the seven outcomes recognized by the NC LINKS program. • Provide referrals of eligible young adults • Enroll all young adults (ages 18-21)with Building Futures program. • Provide employment services through Building Futures to assist young adults in reaching their employment goals. • Refer all young adults who meet criteria for the Rapid Rehousing program to assist in offering safe and stable housing. • Provide transportation for participants via agency and staff vehicles, if available and needed, during normal business hours • Maintain comprehensive case dictation for each participant and share information/data with all collaborative partners in regards to housing, income, and financial management outcomes. • Create, maintain,and submit all required financial documentation to the United Way of the Greater Triangle(UWGT)to receive reimbursement for all services provided by the Collaborative. • Collect financial documentation from Collaborative partners and reimburse each partner for services provided in a timely and accurate manner. • Generate and submit reports on behalf of the Collaborative as required by UWGT • Participate in all meetings with the UWGT to share data and further the goals of this Collaborative Durham Technical • Provide academic counsel,financial aid management, and career Community College resources to participants who are considering enrollment or who are ($4,000) enrolled at Durham Tech i • Provide referrals of eligible young adults • Attend and participate in meetings with the Collaborative at a frequency determined appropriate by the partner agencies • Maintain comprehensive case notes for each participant and share information/data with all collaborative partners in regards to educational outcomes. • Create and submit invoices and other required financial documentation to OCDSS for reimbursement. • Generate and submit reports on behalf of the Collaborative as required by UWGT • Participate in meetings with UWGT to share data and further the goals of this Collaborative Youth Villages • Provide transitional living services and trauma focused CBT to ($4,500) participants
