Roles and Responsibilities Within the Partnership <br /> Partner Roles <br /> Orange County + Act as the lead agency within the collaboration <br /> Department of • Attend and participate in meetings with the Collaborative at a frequency <br /> Social Services determined appropriate by the partner agencies <br /> ($32,000) • Provide primary case management for all participants through the LINKS <br /> program. <br /> • Ensure each participant has access to eligible LINKS funding to promote <br /> the achievement of the seven outcomes recognized by the NC LINKS <br /> program. <br /> • Provide referrals of eligible young adults <br /> • Enroll all young adults (ages 18-21)with Building Futures program. <br /> • Provide employment services through Building Futures to assist young <br /> adults in reaching their employment goals. <br /> • Refer all young adults who meet criteria for the Rapid Rehousing <br /> program to assist in offering safe and stable housing. <br /> • Provide transportation for participants via agency and staff vehicles, if <br /> available and needed, during normal business hours <br /> • Maintain comprehensive case dictation for each participant and share <br /> information/data with all collaborative partners in regards to housing, <br /> income, and financial management outcomes. <br /> • Create, maintain,and submit all required financial documentation to the <br /> United Way of the Greater Triangle(UWGT)to receive reimbursement <br /> for all services provided by the Collaborative. <br /> • Collect financial documentation from Collaborative partners and <br /> reimburse each partner for services provided in a timely and accurate <br /> manner. <br /> • Generate and submit reports on behalf of the Collaborative as required <br /> by UWGT <br /> • Participate in all meetings with the UWGT to share data and further the <br /> goals of this Collaborative <br /> Durham Technical • Provide academic counsel,financial aid management, and career <br /> Community College resources to participants who are considering enrollment or who are <br /> ($4,000) enrolled at Durham Tech <br /> i <br /> • Provide referrals of eligible young adults <br /> • Attend and participate in meetings with the Collaborative at a frequency <br /> determined appropriate by the partner agencies <br /> • Maintain comprehensive case notes for each participant and share <br /> information/data with all collaborative partners in regards to <br /> educational outcomes. <br /> • Create and submit invoices and other required financial documentation <br /> to OCDSS for reimbursement. <br /> • Generate and submit reports on behalf of the Collaborative as required <br /> by UWGT <br /> • Participate in meetings with UWGT to share data and further the goals <br /> of this Collaborative <br /> Youth Villages • Provide transitional living services and trauma focused CBT to <br /> ($4,500) participants <br />
