Purpose of This Memorandum <br /> This Memorandum of Agreement outlines the relationship between United Way of the Greater Triangle <br /> ("United Way" or"UWGT"), who has agreed to provide $60,000 in funding to Orange County, a local <br /> political subdivision of the State of North Carolina, by and through the Orange County Department of <br /> Social Services("Orange County DSS")for the purpose of Orange County DSS serving as a Lead Agency <br /> to implement a new model of integrated service delivery that targets young people aging out of foster <br /> care and those still eligible for LINKS services. <br /> 2. Terms <br /> This Memorandum of Agreement shall take effect on June 1, 2015 and will go for one year, ending on <br /> May 30, 2016.The MOA may be renewed or extended beyond the effective date with the approval of <br /> both parties. <br /> 3. Partnership <br /> The partnership represented by this agreement is committed to working together with and on behalf of <br /> ALL young people aging out of foster care system and those who are eligible for LINKS services. United <br /> Way and the members of this collaborative will create a new integrated service delivery model designed <br /> to increase the stability of these young people as they move, independently into adulthood. <br /> This partnership will develop tools and strategies for implementing, describing and communicating the <br /> common elements of this overall partnership so that a)it can be adapted and implemented in other <br /> communities, and b)additional resources and funding can be secured. <br /> 4. Role of the Lead Agency <br /> As the Lead Agency the Orange County DSS is expected to make sure that the collaborative is effective <br /> and that all participants agree to their roles and responsibilities as outlined in Chart 1.The Lead Agency <br /> is responsible for ensuring the accurate pass through of financial assistance to the appropriate partners. <br /> [CONTINUED TO NEXT PAGE] <br />
