Expense Description Amount Supplies for Volunteer and Social Opportunities Programs: Health and Nutrition Cooking $1,000 Class,HOPE Gardens,Petals with a Purpose,Community Connections Partner Program Rent and Utilities $3,000 Salary,Travel $1,000 Program Services For assistance with this or the following section, please reference the Exhibit A instructions and example, located within the contract and reporting memorandum. Outline the major activities the agency will employ to attain the Anticipated Outcomes below,by June 30,2015. • Health and Nutrition Cooking Class and HOPE Gardens year round weekly class Tuesdays 12:30 2:30 pm, focuses on healthy food choices preparing food during class and Arc individuals use the recipes at home. Cooking class gives back to the community by preparing once a month dessert for the local Meals on Wheels program. HOPE gardens teaches people how to grow organic food and encourages volunteerism. • Petals with a Purpose year round weekly class Wednesdays from 1:00 3:00 pm, fresh flowers donated from Whole Foods using repurposed recycled wine bottles, soup cans creates flower vases that are delivered to people in hospice, senior citizens, St. Joseph's food pantry, the Root Cellar as "A" "R" "C" Acts of Random Compassion,Are individuals giving back. • Community Connections matches a volunteer with an Arc participant and connects them to the community with friendship, shared interests,attending Arc social classes. Anticipated Outcomes The Anticipated Results column must include quantifiable results in the form of number of persons/units served within Orange County, only (all Towns and municipalities). If you use percentages, you must also provide the total number of participants within that measure's description or for an earlier performance measure.
