DocuSign Envelope ID: 76A32744-ADOD-41DA-8l B5-2F4lC532994B <br /> Exhibit A: <br /> Scope of Services <br /> Prepared on 5/4/2015 by the Forum for Youth Investment <br /> for the Orange County Health Department <br /> Period of Performance:May 1,2015—June 30,2016 <br /> Background <br /> Over the past several months, the Forum for Youth Investment (Forum)has been providing Ready by <br /> 211) supports to the Orange County Health Department and Family Success Alliance as they launched a <br /> project to collect better data on the services and supports that children and youth receive in Zones 4 and <br /> 6 and to improve outcomes in those regions. As Orange County prepares to release baseline findings for <br /> said initiative, and given the ongoing Ready by 21 work in nearby Wake County, NC, the parties agree <br /> that maintaining and deepening our collaborative relationship is essential at this time. <br /> As described below, the Forum proposes to continue support collective impact efforts in Orange County <br /> by: preparing for and participating in a full day kick-off event in Fall 2015 (date TBD) organized around <br /> the release of the Family Success Alliance's baseline data reports for Zone 4 and Zone 6. During the <br /> event, the Forum will provide an introduction to and national perspective on collective impact initiatives <br /> for children and youth and facilitate discussions about the course of and goals for the Family Success <br /> Alliance's future work. <br /> Before and after the event, the Forum will continue to provide expert coaching services via phone and <br /> email. Coaching services under this Agreement will be focused on building consensus around and <br /> supporting execution of action steps named during the kick-off event. As appropriate and desired, <br /> coaching time can also be used to discuss and build potential connections to regional collective impact <br /> efforts. <br /> Larry Pasti, Senior Director of Field Services at the Forum, will manage and serve as the lead coach for <br /> the engagement. <br />
