,4, The proposed additional wireless facilities comply with all federal, State, <br />and local safety requirements. <br />The proposed additional facility does not exceed the applicable weight <br />limits for the wireless support structure. <br />d~ All applications for telecommunication facilities that are eligible for <br />administrative approval must contain each of the following components in <br />order to be deemed complete: <br />1~Payment of all applicable fees required by Section 6.18.3ffl of this <br />Ordinance and engineering or consultant fees incurred by the County in <br />evaluating the application. <br />2~A signed statement from the applicant certifying that the applicant's <br />proposed Wireless support structure shall be maintained in a safe <br />manner and in compliance with all conditions of all applicable permits <br />and authorizations without exception, as well as all applicable and <br />,permissible local codes ordinances and regulations, including any and <br />all applicable County State and Federal Laws, rules, and regulations. <br />3~ A signed statement from the applicant certifying that the applicant is <br />duly authorized to do business in the State, and if the applicant is a <br />foreign entity the application must contain a certified copy of the entity's <br />Certificate of Authority from the office of the North Carolina Secretary of <br />State. <br />4)A site plan prepared in accordance with Article XIV of this Ordinance <br />showing the footprint location and dimensions of access drives, <br />landscaping and buffers fencing as well as all other requirements for <br />site plans required by Article XIV. <br />Elevation drawings showing the vertical rendition of the wireless support <br />structure identifying all users attachments, and all related fixtures, <br />structures appurtenances and apparatus, including height above the <br />lowest adjacent pre-existing grade, materials, color and lighting and <br />shielding. <br />,6~The azimuth size and centerline height location of all proposed and <br />existing antennas on the supporting structure. <br />7)The number tape and model of the antenna(s) proposed, along with a <br />copy of the specification sheet(s) for the antennas; <br />8)The age of the existing tower (if any) in years, including the date of the <br />grant of the original permit or authorization for the existing tower; <br />~A description of the type of wireless support structure, e.g. guyed, self- <br />supporting lattice or monopole; <br />10) The make model type and manufacturer of the wireless support <br />structure and the structural design calculations, certified by a <br />Professional Engineer licensed in the State, proving the structure's <br />capability to safely accommodate the facilities of the applicant without <br />change or modification, or if any change or modification of the structure <br />
