50 <br />transmission facilities if the applicant is not a cellular or functional <br />equivalent carrier; <br />7) The actual intended transmission power stated as the maximum <br />effective radiated power (ERP), both in dBm's and watts; <br />8) A statement certifying that the facility and all attachments thereto <br />comply with the conditions of the approved Special Use Permit. <br />9) The legal name, address, phone number, all necessary contact <br />information of the person preparing the application; <br />10) The name, address, phone number, and all necessary contact <br />information of the property owner, if the owner of the structure is <br />different than the applicant; <br />11) The postal address, tax map block and lot number, and parcel <br />identification number of the property; <br />12) A copy of the FCC license applicable for the intended use of the <br />facility. <br />13) The Zoning District and Watershed overlay in which the property is <br />situated; The area of the property on which the structure to be <br />attached to is located, stated both in square feet and acres and <br />perimeter lot line dimensions, and a survey showing the location of <br />all lot lines, metes and bounds, and all easements; <br />14) The location, size and height of all existing and proposed structures <br />on the property on which the structure is located and that is the <br />subject of the Application; <br />6.18.2 Wireless telecommunications support structures and equipment as principal <br />or accessory uses <br />a) Telecommunications wireless support structures_shall be permitted as a <br />principal or accessory use in all districts in accordance with the provisions <br />of this Ordinance as follows: <br />1) On property owned by the County or any public entity except those <br />designated as historic properties or sites. The County may, in its <br />sole discretion as the owner of the property, authorize the <br />application and use of County property after the applicant executes a <br />lease agreement acceptable to the County. <br />2) Telecommunications wireless support structures, as part of existing <br />utility poles shall be permitted as an accessory use. Towers shall be <br />constructed as part of the existing utility poles or as replacements for <br />the existing utility poles. No freestanding towers constructed <br />exclusively for personal wireless services shall be permitted within <br />utility easements. <br />
