Participants also have access to a variety of <br />service options including home care options, retirement <br />benefits, wellness activities, and opportunities for recreation, <br />education, and leisure activities. <br />Information and Assistance <br />Aging <br />$110,791 <br />Provision of information and options counseling through the <br />senior centers; visits to individuals' homes; consultations for <br />assessment of needs and follow -up, including crossover <br />assistance through Social Services. <br />Senior Center Operations <br />Aging <br />$76,494 <br />Grant funds offset a portion of the staff salaries to facilitate <br />operations at the Central Orange Senior Center in <br />Hillsborough and the Seymour Center in Chapel Hill. <br />Transportation (through OPT) <br />Aging <br />$60,418 <br />Daily fixed transportation routes are provided for seniors to <br />attend activities and services at the two multipurpose senior <br />centers in Chapel Hill and Hillsborough that include the <br />daily Senior Lunch Program. <br />In -Home Aide Service <br />Social Services <br />$113,725 <br />Contract for personal care and home management tasks to <br />forestall long -term placement. <br />Adult Day Health <br />Senior Care of <br />$52,580 <br />Assistance and support to unpaid caregivers through the <br />Orange County, Inc. <br />supervision of frail elderly participants in a community facility <br />setting under the adult day health and social models. <br />Total HCCBG Funding Plan <br />$553,104 <br />
