3. the name, address, location, and telephone num- <br />ber of the location to which the animal will be taken for the <br />following five (5) days; <br />4. the name, address, and telephone number of the <br />collector;[,- <br />5. the name, address, and telephone number of the <br />person surrendering the animal. <br />b. A written report containing the above information <br />shall be delivered by the person collecting the animals to the <br />Animal Shelter within twenty -four (24) hours of the surrender of <br />the animal. <br />C. A record shall be kept of the disposition of every <br />animal which is collected. <br />d. Every collector shall maintain a copy of all <br />receipts and disposition records for one (1) year. <br />B. A Quality Assurance program to ensure adherence to these <br />standards shall be carried out within the Animal Control <br />Division. <br />C. The Animal Control Director shall be responsible for the <br />full and proper application of this policy. Questions concerning <br />the applicability or interpretation of this policy shall be the <br />responsibility of the Animal Control Director. <br />XXI <br />Section [4�x3q� - penalties.
