Explanation of Services to be provided and to whom (include SIS Service Code): <br />The Contractor provides an oven entry and exit training program. consistins of <br />integrated basic skills and vocational training, for up to five students. The Contractor <br />must place in emploXment at least 70% of the Work First trainees who complete the <br />program, and place 70% of those employed in jobs related to the skills and training <br />acquired in their course of study. The Contractor is required to meet all goals and <br />outcomes listed in Attachment N. <br />C. Rate per unit of Service (define the unit): <br />1. If Standard Fixed Rate, Maximum Allowable, (See Rates for Services Chart) <br />2. Negotiated County Rate. <br />Based on tuition schedule up to a maximum of $30,000 <br />D. Number of units to be provided: Tuition for up to five students <br />E. Details of Billing process and Time Frames; The County will reimburse the Contractor <br />for services described in this contract up to the budgetary limits of the contract allotment. <br />The County will reimburse the Contractor the total tuition amount as specified in <br />Attachment M, less 90% of any estimated Pell grant, for each trainee, based on course <br />selection at the time of enrollment. For reimbursement the Contractor must submit an <br />original and two copies of an invoice bathe fifth of the month following the month of <br />Contract-Scope of Work (06/04) Page lof 2 <br />
