9 <br />NAME OF BOARD /COMMISSION: Adult Care Home Community Advisory <br />Committee <br />Report Period: January 2014 — December 2014 <br />ORANGE COUNTY ADVISORY BOARDS AND COMMISSIONS ANNUAL <br />REPORT <br />The Board of Commissioners welcomes input from various advisory boards and <br />commissions in preparation for its annual planning retreat. Please complete the <br />following information, limited to the front and back of this form. Other <br />background materials may be provided as a supplement to, but not as a <br />substitute for, this form. <br />Board /Commission Name: <br />Orange County Adult Care Home Community Advisory Committee (ACH <br />CAC) <br />Person to address the BOCC at work session - if applicable - and contact <br />information: <br />Maxwell O. Mason - Advisory Committee Chair <br />Address: 821 Tinkerbell Rd., Chapel Hill, NC 27517, Tel: 919 - 649 -7937 <br />Primary County Staff Contacts: Janice Tyler, Director, County Department on <br />Aging, and Mary Fraser, Aging Transitions Administrator, County <br />Department on Aging. Charlotte Terwilliger, Long -Term Care <br />Ombudsperson, Triangle J Council of Governments, also provides staff <br />support to the committee. <br />How many times per month does this board /commission meet, including any <br />special meetings and sub - committee meetings? <br />Bi- monthly committee business meetings and quarterly subcommittee <br />visits to long term care facilities. Issue specific subcommittees meet on an <br />ad hoc basis. <br />Brief Statement of Board /Commission's Assigned Charge and Responsibilities. <br />• Stay informed about living conditions for residents of long term care <br />(LTC) assisted living facilities and homes. <br />• Make official unannounced quarterly visits to each assisted living <br />facility in county, and unannounced annual visits to assisted living <br />
