16 <br />Provided leadership to the Orange County Long -Term Care Learning Collaborative by <br />sponsoring the Long Term Care Quality Service Award process. Three local long -term care <br />organizations received Quality Service Awards in 2014 for new activities programming. <br />Provided financial and technical support to 7 Medicaid - sponsored nursing homes, assisted <br />living facilities, and adult day care programs to implement a Music In My Mind program for <br />residents with dementia. <br />Made progress in implementing the Department's Communication Plan by expanding the <br />information provided on the OCDOA website, making the website more "senior friendly ", and <br />initiating a Facebook presence. <br />List of Specific Tasks, Events, or Functions Performed or Sponsored Annually. <br />• Provide Orange County representation to the Triangle J Area Agency on Aging and the Senior <br />Tar Heel Legislature. <br />• Support the Chatham - Orange Community Resource Connections and the development of its <br />partnerships with local service providers including the pilot hospital transitions project as well <br />as the Senior Health Advocacy & Resource Partners workgroup. <br />• Hold an annual Master Aging Plan update meeting for the public and partner agencies. <br />• Make recommendations to the BOCC of potential legislative issues to be included in the <br />Orange County legislative agenda. <br />Describe this board /commission's activities /accomplishments in carrying out BOCC goal(s) /priorities, <br />if applicable. <br />If your board /commission played the role of an Element Lead Advisory Board involved in the 2030 <br />Comprehensive Plan preparation process, please indicate your board's activities /accomplishments as <br />they may relate to the Comprehensive Plan's goals or objectives. <br />(The Element Lead Advisory Boards include: Planning Board, EDC, OUTBoard, Commission for the <br />Environment, Historic Preservation Commission, Agriculture Preservation Board, Affordable Housing <br />Board, Recreation and Parks Advisory Council) <br />Identify any activities this board /commission expects to carry out in 2015 as they relate to established <br />BOCC goals and priorities. <br />If applicable, is there a fiscal impact (i.e., funding, staff time, other resources) associated with these <br />proposed activities (please list). <br />Continue with Master Aging Plan Year Three Implementation Plan through June 30, 2015 and <br />prioritize strategies for Year Four initiatives. <br />What are the concerns or emerging issues your board has identified for the upcoming year that it <br />plans to address, or wishes to bring to the Commissioners' attention? <br />• Transportation accessibility, especially in the rural parts of the county <br />• Walkability and navigation barriers throughout the county /towns <br />• Need for more affordable and accessible housing, especially rental units <br />• Poor quality of many long -term care facilities operating within the county <br />• Mal- nutrition among many poor, older adults — limited "meals on wheels" availability in northern <br />part of the county. <br />
