We will submit to HUD the Five Year <br /> Consolidated Plan through IDIS Three copies and an electronic <br /> copy will be provided to the County and Town of Chapel Hill <br /> B. FY 2015 Annual Action Plan: <br /> We understand the types of services required to prepare the FY <br /> 2015 <br /> Annual Action Plan utilizing the latest eCon Planning Suite in IDIS The <br /> Annual Action Plan will contain the following elements: <br /> • Form HUD-424. Completion of the new HUD form for signature <br /> by the appropriate local Officials, Other resources that are utilized <br /> for leveraging and match will be listed <br /> • Executive Summary. Our staff will prepare the Executive <br /> Summery which will identify the available funds, strategies and <br /> objectives and activities to be unde;naken,. <br /> • Summary of Priorities, Goals, Budgets and Anticipated <br /> Accomplishments. We will prepare a listing of the five year <br /> strategic initiatives, which initiatives will be under taken in the <br /> annual action plan,and the number of persons benefiting <br /> • General Questions. Our staff will address the HUD general <br /> questions in the latest IDIS format which will include a description <br /> of the geographic areas of the County,what was the basis used to <br /> allocate funds by geographic area,, what hinds being used to <br /> address undersenred needs,and wha t resources are available <br /> • Managing the Process. We will describe what agencies are <br /> administering programs the process used to develop the plan,and <br /> ways to enhance coordination among agencies <br /> • Citizen Participation. Our staff will describe the citizen <br /> participation process used, prepare the notices to be published in <br /> the local newspaper, attend the local governing body's meeting to <br /> adopt the Plan,and prepare a summary of any citizen comments. <br /> • Institutional Structure. Our staff will describe what actions the <br /> County proposes to undertake to improve the institutional structure <br /> • Monitoring. We will describe what actions the County will utilize to <br /> monitor its projects and sub-recipients. <br /> • Lead Based Paint. A description of the extent of lead based paint <br /> hazards and what actions are being undertaken to reduce and'or <br /> abate lead based paint <br /> • Specific Housing Objectives. Our staff will prepare the listing of <br /> specific housing objectives,priority status and available resources <br /> • Needs of Public Housing. We will meet with the staff of the local <br /> Housing, Authority and review their plans and needs of public <br /> housing,and the housing authority's performance rating <br /> • Barriers to Affordable Housing. We will prepare a narrative on <br /> the barriers to affordable housing and what actions are needed to <br /> remove,reduce,and eliminate barriers to affordable housing <br /> • Specific Homeless Prevention Elements. We will address the <br /> areas and issues, including: what sources of funds are 'available; <br /> the extent of homelessness and chronic homelessness in the area; <br /> programs are in place or proposed to prevent homelessness; the <br />
