Page 2 of 3 <br /> Agreement Between <br /> Orange County Health Department and <br /> The University of North Carolina at Chapel Hill <br /> Department of Obstetrics and Gynecology <br /> 2 . To provide obstetrics and gynecologic consultation <br /> regarding patients referred by the nurse <br /> practitioner. <br /> 3 . To assist in the dispensing of prenatal medications. <br /> 4. To consult with the senior attending at UNC Hospital <br /> about patient_referrals and high risk patients. <br /> 5. To attend the post clinic conference. <br /> All care rendered will be appropriately documented in writing. <br /> B. UNC Hospitals and The University's Department of Obstetrics <br /> and Gynecology <br /> proposes to contract for 50 clinics per year (there being no clinic during <br /> Thanksgiving week and allowance being made for one additional week relative <br /> to an agreed upon holiday) . Each clinic will require 3 1/2 hours of <br /> resident time and 3/4 hour travel time. The cost per hour is $40.00. For <br /> fifty (50) clinics the total cost per year would be $8,500.00. University <br /> will bill OCHD monthly and OCHD will pay the University within 30 days of <br /> receipt of University's invoice. <br /> C. Upon request, The University will provide evidence of <br /> Professional Liability coverage by the UNC Hospitals in the amount of at <br /> least $1, 000, 000.00 on all duly appointed members of the duly appointed <br /> Housestaff under the self insurance program of the University of North <br /> Carolina at Chapel Hill, in order to cover financial liability which may <br /> arise from the negligence or wrongful acts or omissions of such <br /> residents/housestaff while on rotation at Orange County Health Department. <br /> D. This Agreement shall run for a period of one (1) year from the <br /> 1st day of July, 1994 to the 30th day of June, 1995 and shall be renewed <br /> from year to year thereafter upon written notice executed by both parties. <br /> E. This Agreement or its renewals may be terminated at any time <br /> without penalty by either party provided that written notice of such <br /> termination is furnished to the other party at least 90 days prior to <br /> termination. In the event of such termination any payment due shall be <br /> prorated to the date of termination. <br />
