P <br /> ACTION AGENDA ITEM ABSTRACT <br /> Meeting Date: June 28, 1994 <br /> SUBJECT: <br /> Community Based Public Health Initiative ('CBPHI) <br /> Sub contract with JOCCA. <br /> DEPARTMENT: HEALTH PUBLIC HEARING: Yes % No <br /> ATTACHMENT(S) : INFORMATION CONTACT: <br /> HEALTH DIRECTOR'S OFFICE X2412 <br /> Agreement Between OCHD and JOCCA <br /> TELEPHONE NUMBER: <br /> Hillsborough - 732-8181 <br /> Chapel Hill - 968-4501 <br /> Mebane - 227-2031 <br /> Durham - 688-7331 <br /> PURPOSE: <br /> To promote community involvement in planning community based interventions to <br /> improve the health of minority populations in selected Orange County communities <br /> through a sub contract of Kellogg Foundation funds from the Health Department to <br /> the Joint orange Chatham Community Action Agency. <br /> BACKGROUND: <br /> The Community Based Public Health Initiative (CBPHI) was accepted by the Board of <br /> commissioners on January 19, 1993. At that time the Commissioners voted to: (1) <br /> accept the grant funds from the Kellogg Foundation, via the UNC School of Public <br /> Health, to develop these CBPHI; (2) establish time limited positions for a full <br /> time Health Educator I and a half time office assistant; and (3) authorize orange <br /> County to enter into sub contracts with JOCCA and OCCHS. <br /> The project follows the Kellogg fiscal year and the initial contract was from <br /> October 1, 1992 through September 30, 1993. Renewal of the contract has been <br /> delayed pending completion by JOCCA of the reports required by the sub contract. <br /> These have been completed and JOCCA has requested expeditious processing of the <br /> contracts so that funds for the fiscal year ending September 30, 1994 can be <br /> distributed. <br /> JOCCA has successfully completed its workplan and has been an active participant <br /> in the partnership. It has fostered the incorporation of the Efland Development <br /> Association, worked to include the cheeks Home owners Association in the <br /> coalition and participated in all of the community based initiatives of the <br /> orange county coalition. <br /> RECOMMENDATION: <br /> The manager recommends that the sub contract be approved and the chair authorized <br /> to sign the sub contract. <br />
