Attachment IA Amendment II - Contract#0887 Orange County Health Department The contractor will provide families and children,from birth to 5, having an identified risk or who are already identified with any developmental delay, social/emotional disorder or chronic illness,access to preventive,specialized and support services. Contact will be made through the home,day care,telephone and clinical visits at least quarterly; or more frequently if needed, to reassess needs and do follow-ups or referrals. A social worker will also be hired,trained,and ready to carry a case load by September 1, 1994.
