26 <br /> 5. Project #5: Social Worker for AFDC/Working and Transitional Families <br /> a. Type of activity: Social Work Position/Funding <br /> b. Description of activity: This project will provide a full-time position to provide social <br /> work services to families who receive child care subsidy because they are working and <br /> receiving AFDC, or because they are receiving Transitional benefits. These families currently <br /> do not receive any social work support. <br /> c. Agencies/organizations involved, and description of their collaboration: Orange <br /> County Department of Social Services <br /> d. Target population: AFDC/Working and Transitional families <br /> e. Expected outcomes, including time-frame: <br /> +Coordinate services for 200 children by June 30, 1995. <br /> •Provide parent information on how to choose quality child care. <br /> •Encourage use of regulated providers. <br /> •Negotiate arrangements with providers. <br /> •Educate providers regarding subsidy systems. <br /> •Make payments for approximately 120 children/month for child care. <br /> •Develop and initiate vendor payments for Transitional Child Care. <br /> •Provide social work services for 150 families by June 30, 1995. <br /> •Coordinate payments and services within the agency and the day care community. <br /> •Outreach to communities and families to improve knowledge about and access to services. <br /> •Work with client groups and/or individuals to gain information on the best way to provide <br /> services. <br /> •Develop an accountability system which will help in the measurement of outcomes. <br /> Improve public and agency awareness of services. <br /> f. Implementation plan, including time-frame: The position will be filled by May 15, <br /> 1994, training will be provided, and the social worker will assume a caseload by 6/l/94. <br /> g.
