Howes, Secretary D F= H N 11 <br /> May 9, 1994 <br /> MEMORANDUM <br /> TO: Health Directors: Health Check Outreach Projects <br /> FROM: Tom Vitaglione, Chief qq�_�V. <br /> Children and Youth Section <br /> SUBJECT: Contracts for FY 94-95 <br /> As promised, each county participating in the Health Check Outreach Project is <br /> eligible to receive a $10,000 contract for FY 94-95. Enclosed is the activity <br /> budget page and the contract addendum page in this regard. <br /> Please complete as appropriate and return both pages to the regional office that <br /> serves your county. If you are able do so quickly, we can add these pages to the <br /> activity budgets you have already submitted for other services for FY 95. <br /> Please recall that the $10,000 is intended as "bridge" money as Health Check <br /> Outreach becomes a cost-based, reimbursable service. It is still Medicaid's <br /> intention to begin reimbursing for this service on July 1. The $10,000 should <br /> cover your expenses while the service becomes self-sustaining. <br /> We appreciate the efforts of your staffs in assisting us with the entire Health <br /> Check effort. The new data system and public awareness materials - all based on <br /> their input - are nearing implementation.
