2 <br /> Since the Office Assistant and outreach Social Worker were hired <br /> December 20, 1993, the following outcomes have been accomplished: <br /> 1. Nearly 30 contacts have been made with other providers. <br /> 2. Both workers have attended training programs and received <br /> on-site training from State consultants. <br /> 3. Contacts and linkages have been made with other agencies <br /> such as Head Start, Women's Infants and Children Nutrition <br /> Program, Orange Chatham Comprehensive Health Service, <br /> Carolina Access and the DSS Medicaid staff, and the <br /> Community-Based Public Health Initiative. <br /> 4 . Patient records have been established on 52 EPSDT cases. <br /> 5. 54 patient reminder letters have been sent. <br /> 6. 82 patient appointment verifications have been done. <br /> 7 . 91 patient related contacts have occurred. <br /> These positions were approved by the Board through June 30, 1994 . The <br /> Department of Health, Environment and Natural Resources (DEHNR) and the <br /> Division of Medical Assistance (DMA) have been working on developing a <br /> plan for ongoing funding for this program through Medicaid <br /> reimbursement for outreach services. We are awaiting word on what the <br /> plan will be. As soon as we are informed of the reimbursement plan, we <br /> will develop a local response and present a request to the Board for <br /> continuation of the program and the positions beyond September 30, <br /> 1994. <br /> RECOMMENDATION(S) : The Manager recommends that the Commissioners <br /> accept the $10,000 contract, approve continuation <br /> of the permanent time-limited Office Assistant and <br /> Social Worker positions through September 30, 1994 , <br /> and authorize the Chair to sign the contract <br /> amendment and the budget. <br />
