3 <br /> Referral, Follow-up & Monitoring, Education & Counseling <br /> F ' <br /> activities with eligible women. These services will be <br /> coordinated with those provided by the Department of Social Work <br /> and other Hospital Staff. <br /> The person (or persons) whose services are to be provided <br /> pursuant to this Agreement is (are) for all purposes an employee <br /> of Orange County Health Department. However Social Workers <br /> employed by the County to fulfill this function must apply for and <br /> be certified as qualified to perform patient care activities in <br /> the Hospital.. The delineation of these privileges shall be set <br /> forth in an individual agreement between the Hospital and the <br /> Worker. <br /> This Agreement shall run for a period of one year, from the <br /> 1st day of July, 1993, to the 30th day of June, 1994 . It is the <br /> intention of the parties that this Agreement be reviewed annually <br /> and may be renewed. It may be terminated by either party upon 60 <br /> days written notice. <br /> This Agreement contains the entire understanding of the <br /> parties and shall not be altered, amended, or modified, except by <br /> an agreement in writing executed by the duly authorized officials <br /> of both parties. <br /> #1 LM AgrUNCOC <br />
