2 <br /> STATE OF NORTH CAROLINA UNCH # <br /> COUNTY OF ORANGE <br /> AGREEMENT BETWEEN <br /> The University of North Carolina Hospitals <br /> and <br /> Orange County <br /> THIS AGREEMENT, made and entered into this the 1st day of <br /> July, 1993, by and between University of North Carolina Hospitals <br /> (hereinafter referred to as the Hospital) for its Department of <br /> Social Work and Orange County (hereinafter referred to as the <br /> County) for its Health Department <br /> WITNESSETH: <br /> a WHEREAS, the Hospital and County wish to collaborate in <br /> addressing and correcting barriers that negatively affect <br /> pregnancy outcomes; and <br /> WHEREAS, the parties desire to execute this agreement to <br /> delineate this collaboration. <br /> NOW, THEREFORE, the parties hereby agree as follows: <br /> 1. The County agrees to make available the services of a <br /> Maternity Care Coordinator, without charge, to collaborate with <br /> Hospital staff in providing care to pregnant Orange County women. <br /> 2. The Hospital agrees to provide shared office space, <br /> accessibility to office equipment and telephone service, and <br /> liaison services from the Chief Clinical Social Worker, OB/GYN. <br /> 3 . The Maternity Care Coordinator shall provide Outreach, <br /> Recruitment, Assessment, Service Planning, Coordination & <br />
