i 5 <br /> Because members of the Foundation's Distributions Committee and staff review each proposal, <br /> please submit six collated copies of the following: <br /> • A completed copy of the enclosed application form, signed by the Chief Officer of the <br /> Board (attach to the top of each packet) <br /> • The narrative proposal, not to exceed three single-sided pages <br /> • A list of Board members with their primary professional or community affiliations <br /> • A descriptive, line-item budget for the proposed project; as specific as possible <br /> In addition, your proposal should include one copy of the following attachments: <br /> • Your organization's most recent overall operating budget, audited if possible <br /> • A copy of your most recent IRS Form 990 (not applicable to government agencies and <br /> religious organizations) <br /> • IRS tax determination letter or other proof of tax-exempt status (not applicable to <br /> government agencies and religious organizations) <br /> In the interest of fairness and consistency, additional attachments beyond those requested will <br /> be discarded.
