BOARD OF ORANGE COUNTY COMMISSIONERS (BOCC) <br />GOALS — 2015 UPDATE <br />FOR DISCUSSIONS PURPOSES <br />GOALS/PRIORITIES <br />ONGOING EFFORTS <br />SPECIFIC ACCOMPLISHMENTS <br />• Objective 4.2: Improve the quality of programs <br />and services provided to residents of long -term <br />care facilities, nursing homes, assisted living <br />facilities, and family care homes in Orange <br />County. <br />• Strategy 4.3.2: Help older adults' access <br />needed social service programs. <br />• Objective 6.1: Promote aging preparedness so <br />that the Orange County community and its <br />residents may be better able to transition to <br />senior living. <br />• The Long Term Care (LTC) Learning <br />Collaborative was created with a focus on <br />implementing person centered care in our <br />LTC facilities. <br />• Seminars have been developed and offered <br />to LTC staff and administrators. <br />• Conducted a survey of LTC administrators <br />about the barriers and incentives for <br />implementing person- centered practices. <br />• The Long Term Care Quality Service <br />Awards were developed and were awarded <br />at a BOCC meeting. <br />• The Music in My Mind program is being <br />implemented in nine LTC facilities in our <br />county. <br />• Our DSS /Aging Social Workers are <br />available at both of the senior centers and <br />we pride ourselves in being the "One Stop <br />Shop" for aging services in the county. <br />• The county's Veteran Affairs Officer is <br />now offering regular office hours at both of <br />
