� <br /> ' <br /> | <br /> STATE OF NORTH CAROLINA <br /> COUNTY OF ORANGE AGREEMENT <br /> THIS AGREEMENT, made and entered into this first day of July, 1995, by and between <br /> the University of North Carolina at Chapel Hill, hereinafter referred to as'Me University,"for <br /> its Department of Pediatrics in the School of Medicine,hereinafter referred to as the <br /> "Departmene'and the Orange County Health Department,hereinafter referred to as the <br /> WHEREAS, the OCHD is the operator of two clinics in Carrboro,North Carolina and in <br /> Hillsborough,North Carolina and desires the Department to provide medical care for children <br /> and adolescents at OCHD; and <br /> . WHEREAS, the Department desires to participate in the provision of medical care for <br /> children and adolescents at OCHD; and <br /> WHEREAS,performance of the activity described herein is consistent with the <br /> University's goals of research, teaching, education and public service. <br /> NOW, THEREFORE,in consideration of the premises and the following mutual <br /> covenants and conditions and any sums to be paid, the OCHD and the University agree as <br /> 1. A pediatrician from the University will provide pediatric services in the OCHD's clinics <br /> for an average of twelve (12) hours each week except for 10 weeks during the summer when the <br /> time will average 7.5 hours/week due to the suspension of the adolescent clinic in Carrboro. <br /> Since the clinic does not operate for 12-13 weeks, this would allow for 9 - 13.5 hours of paid <br /> planning time over the summer. The total number of hours covered in the contract is 579 hours. <br /> This time will cover preparation time for and attendance at case conferences and continuing <br /> education meetings on the second Monday of each month as well as on-site attendance at the <br /> following clinics. <br /> Tuesday Adolescent Clinic Carrboro 12:30prn- 5:00 prn <br /> Wednesday Child Health Clinic Hillsborough 9:30 am- 11:30 am <br /> Wednesday Adolescent Clinic Hillsborough 12:30 prn- 5:00 prn <br /> 2. The pediatrician's functions will be to: <br /> a. Provide medical direction to the child health program at the OCHD and the <br /> school health programs in both school systems in the county and medical consultation to both <br /> OCI-ID and school staff working in the school health program. <br /> b. Provide medical services for children. <br /> | <br />
