10 <br /> The Healthy Carolinians 2000 Task Force <br /> Like many communities across the state and nation,yours is considering to launch a <br /> health improvement program.We believe that individuals and families are the key to your <br /> success;only through their commitment to live longer,healthier lives can a community achieve <br /> improved health.We also believe that people are more likely to work toward objectives that <br /> they have set for themselves.By developing a Healthy Carolinians 2000 Task Force,your com- <br /> munity will be able to estabish the community-based objectives necessary to improve its health. <br /> The Healthy Carolinians 2000 Task Force will: <br /> • Inform local leaders about health problems that can be improved by wellness promo- <br /> tion and disease prevention. <br /> • Create an ongoing,community-oriented process for priority setting and action plan- <br /> ning. <br /> • Select and tailor objectives from the list recommended by the state level Task Force. <br /> • Serve as the conduit for funding of cooperative health promotions. <br /> Healthy Carolinians 2000 activates the community to work in a concerted,collaborative <br /> approach It usually involves the following steps: <br /> • Organizing a community group. <br /> • Collecting and analyzing data. <br /> • Identifying health problems and setting priorities or targeting problems. <br /> • Identifying relevant community resources and barriers. <br /> • Developing an action plan. <br /> • Starting the action plan. <br /> • Assessing or evaluating progress. <br /> Several models for the community exist including traditional community development <br /> models, the Assessment Protocol of Excellence in Public Health (APEX PH) and the Planned <br /> Approach to Community Health (PATCH) (See Appendix 7 and 8).Remember that working <br /> with the community is an art and science requiring a functional community group that main- <br /> tains creativity and consensus building. <br /> I <br />
