DocuSign Envelope ID: ECE511A0-1745-4353-8DDA-483D182AA9FD <br /> The ArtsCenter <br /> �I:=, lips>>iriner, <br /> ARTIST SERVICE AGREEMENT <br /> Agreement made on November 20, 2014 between Virginia Queen Danford (hereafter known as <br /> the "Artist") and The ArtsCenter, Inc (hereafter referred to as "TAC"). It is mutually agreed <br /> between the parties as follows: <br /> 1. The Artist will render services to TAC as follows: <br /> • To conduct: <br /> o One (1) five day Artist Residency with the four Fifth Grade classes at New Hope <br /> Elementary <br /> • For the above services, TAC will pay the artist the designated sum of $1650, plus <br /> reimbursement for supplies. The total fee breakdown is as follows: <br /> ® $ 300/day for 5 days = $1500 <br /> ■ $150 total for planning & evaluation sessions <br /> ■ Up to $100 total in supplies <br /> 2. TAC agrees to provide the Artist with the following services: <br /> o A company check within 30 days for all services at the completion of the term of <br /> the contract. <br /> 3. In compliance with North Carolina law, The ArtsCenter is required to maintain Workers' <br /> Compensation insurance for all Instructors. Therefore, henceforth, The ArtsCenter will withhold <br /> 0.77% (or 0.0077) of each payment made to an instructor. (For example, if an Instructor is <br /> owed $200, The ArtsCenter shall deduct $1.54 from this total). Except that if any Instructor <br /> who has personally procured Workers' Compensation insurance offers proof of this to the <br /> Director of Children's & Family Programs, The ArtsCenter will not withhold the above <br /> percentage from its payments to that Instructor. <br /> 4. All attached riders are part of this contract and must be signed by both parties or the contract <br /> will be considered null and void. <br /> 5. In the case of inclement weather or any other act of God beyond either party's control which <br /> does or would significantly impact on the ability of either party to perform or present under <br /> this contract, every attempt will be made to reschedule to the first date convenient to both <br /> parties. Should both parties fail to establish a date to reschedule, the above-stated <br /> obligations will be canceled and neither party shall be able to make a claim for damages or <br /> breach of contract. <br /> 6. Due to concerns imposed on TAC by our insurance company in reference to child safety, TAC <br /> now requires background checks on all of our teaching artists that work with children ages 18 <br /> years old and under. Your social security number and date of birth will be used for this <br /> reason. <br />
