12 <br />7) The actual intended transmission power stated as the <br />maximum effective radiated power (ERP), both in dBm's <br />and watts: <br />8) A statement certifyina that the facility and all attachments <br />thereto comply with the conditions of the. approved Special <br />Use Permit. <br />9) The leaal name. address. phone number, all necessary <br />contact information of the person preparina the application• <br />101 The name, address, phone number, and all necessary <br />contact information of the property owner if the owner of the <br />structure is different than the applicant• <br />11) The postal address. tax map block and lot number, and <br />parcel identification number of the property: <br />12) A copy of the FCC license applicable for the intended use of <br />the facility. <br />131 The Zonina District and Watershed overlay in which the <br />property is situated; The area of the property on which the <br />structure to be attached to is located stated both in sauare <br />feet. and acres and perimeter lot line dimensions, and a <br />survey showina the location of all lot lines, metes and <br />bounds and all easements• <br />14) The location. size and heiaht of all existina and proposed <br />structures on the property on vvhich the structure is located <br />and that is the subject of the Application; <br />6.18.2 Telecommunications mss-wireless support structures and equipment as <br />principal or accessory uses <br />a) Telecommunications fewer-s wireless support structures -shall be <br />permitted as a principal or accessory use in all districts in accordance <br />with the provisions of this Ordinance as follows: <br />1) On property owned by the County or any public entity except <br />those designated as historic properties or sites. The County may, <br />in its sole discretion as the owner of the property, authorize the <br />application and use of County property after the applicant <br />executes a lease agreement acceptable to the County. <br />2) Telecommunications towers wireless support structures as part of <br />existing utility poles shall be permitted as an accessory use. <br />Towers shall be constructed as part of the existing utility poles or <br />as replacements for the existing utility poles. No freestanding <br />towers constructed exclusively for personal wireless services shall <br />be permitted within utility easements. <br />3) The placement of a new telecommunications fewer wireless <br />support structures in any zoning district must be approved as a <br />Special Use, except as permitted in the Master <br />
