EXHIBIT `°A" <br /> Scope of Services—FY 2014 - 15 <br /> Outside Agency Performance Agreement <br /> Agency Name: Ligo Dojo Of Bttdo Karate <br /> Program Name: Young Warriors Program <br /> Funding Award: $2,000 <br /> Outline how the agency will spend Orange County's funding award. <br /> Expense Description Amount <br /> Employee compensation $1,025 <br /> Rent & Utilities $834 <br /> Su Pp lies & eq U pill ent(uniforms, safety gear) $125 <br /> Program Services <br /> For assistance with this or the following section, please reference the Exhibit A instructions and example, located within the <br /> contract and reporting memorandum. Outline the major activities the agency will employ to attain the Anticipated Outcomes <br /> below,by June 30,2015. <br /> • Young Warriors Karate - 32 referred youth will participate in 2 to 5 two-hour classes per week. <br /> Court referred youth are asked to complete 20 classes in approximately 10 weeks. We encourage <br /> all referred youth to continue their karate training beyond required commitments and offer <br /> reduced rates to families in need. <br /> Anticipated Outcomes <br /> The Anticipated Results column must include quantifiable results in the form of number of persons/units served within Oran 2e <br /> County, onh (all Towns and municipalities). If you use percentages, you must also provide the total number of participants <br /> within that measure's description or for an earlier performance measure. <br /> Performance Measures Anticipated <br /> Results <br /> 40% Participants successfully/satisfactorily completing the program will have no new 12 <br /> complaints in the 12 months following com letion. <br /> 60% Participants completing the program will demonstrate a reduction in problem 19 <br /> behaviors for which they were referred by termination. <br /> 50% of children admitted will participate in a promotion test at the end of their term(s) 16 <br /> and will receive the validation of new belt color/promotion. <br /> 50% Participants completing the program will demonstrate improvement in targeted 16 <br /> skills asspecified in the individual service plan by termination. <br /> Certified by: 0 Title: _ Date: S <br /> rovider's Signature) <br />
