Y � <br /> 18 <br /> Appendix Number Three: Evaluation <br /> Formal evaluation will occur along three main dimensions and consist primarily offive <br /> different mechanisms. These are as follows: <br /> A. Dimensions: <br /> 1. Individual: Is the program offering services that meet the expressed needs of community <br /> parents/families? Are participants pleased with the social(i.e., tone) and procedural(i.e., <br /> accessibility) aspects of the service? Do parents feel valued and respected and comfortable, <br /> offering feedback? <br /> 2. Service: Have the services been successful in achieving identified outcomes for community <br /> parents/families? Do significant gaps in service still exist? <br /> 3. System: Are efforts to coordinate the services provided by community agencies efficient, <br /> inclusive, and collaborative? Are agencies satisfied with the terms of the collaborative agreement, <br /> communication patterns between program coordinator and service providers, <br /> B. Mechanisms: <br /> 1. Collection of baseline data on participatory families. <br /> 2. Self-report surveys: <br /> For parents: 1)needs evaluation to inform initial design of program <br /> 2) "pre" and "post" measures of personal attitudes, feelings, competencies, <br /> practices, and needs <br /> 3) "end of course" ratings of service providers, convenience value, <br /> suggestions for improvement <br /> For providers: 1)needs evaluation to inform initial agencies design of program <br /> 2) "end of course" ratings of service, suggestions for improvements <br /> 3. Program record keeping: demographic information recorded for all participants <br /> 4. Agency record keeping: anonymous group information on participants, such as substantiated <br /> CPS Reports, foster placements, enrollment in drug treatment programs(used only to compute <br /> group statistics) <br /> 5. Ongoing, informal exchange between participants and providers <br />
