EXIMIT"A" <br /> Scope of Services—FY 2014- 15 <br /> Outside Agency Performance Agreement <br /> Agency Name: Empowerrment, Inc <br /> Program Name: Affordable Housing Property Management Program <br /> Funding Award: $20,000 <br /> Outline how the agency will spend Orange County's funding award. <br /> Expense Description Amount <br /> Support for the property management program specifically the property manger position $20,000 <br /> that is responsible for advertising units,turn over of units,and couseling potential and <br /> current tenants. <br /> Program Services <br /> For assistance with this or the following section, please reference the Exhibit A instructions and example, located within the <br /> contract and reporting memorandum. Outline the major activities the agency will employ to attain the Anticipated Outcomes <br /> below,by June 30,2015. <br /> • Increase affordable rental inventory <br /> • Provide couseling for potential and current rental clients <br /> • Work closely with local churches , non profits and housing agencies like DSS, IFC, Community <br /> EmPOWERment Fund and to find affordable housing for Orange County residents. <br /> Anticipated Outcomes <br /> The Anticipated Results column must include quantifiable results in the form of number of persons/units served within Oran e <br /> County, only (all Towns and municipalities). If you use percentages, you must also provide the total number of participants <br /> within that measure's description or for an earlier performance measure. <br /> Performance Measures Anticipated <br /> Results <br /> 1. Increase affordable rental stock by two. 2 <br /> 2. Provide housing for 2 homeless families/individuals. 2 <br /> 3. Provide housing for 2 families threatened to be homeless: section 8 clients. -- 2 <br /> 4. Percentage of HQS inspection completed. 95 <br /> 5. Facilitate 2 tenant association meetings. 2 <br /> 6. Facilitate 2 meetings of local non profits,churches,government entities around 2 <br /> affordable rental units. <br /> 7. Attend training classes for property manager and/or housing couseling-2 per year. 2 <br /> 8.
